When you visit an in-network provider, your annual physical is covered at 100% of the Plan Allowable amount, with no copayment required. That means you can get your yearly check-up without worrying about added costs.

If you choose to see an out-of-network provider, the Plan will still pay 40% of the Plan Allowable amount. While this may result in higher out-of-pocket expenses, it gives you the flexibility to choose the provider that works best for you.
